Bones are constantly being remodeled every day, and calcium moves in and out of them. In children and adolescents, the body builds new bone faster than it breaks down old bone so total bone mass increases. This continues until about age 30, when new bone formation and old bone breakdown start occurring at about the same rate. In older adults, especially in post-menopausal women, bone is broken down at a faster rate than it's built. If calcium intake is too low, this can contribute to osteoporosis.

According to the National Institutes of Health, half of all women over age 50 and a quarter of men older than age 50 will break a bone due to osteoporosis. Post-menopausal white and Asian women are at the highest risk for osteoporosis. About 25% of women with osteoporosis will develop a vertebral deformity, and 15% will break a hip. Osteoporosis also causes broken hips in men, although not as often as in women. Hip fractures are associated with an increased risk of death within the year after the bone break.


The U.S. Preventive Services Task Force recommends a bone density screening by DXA in all women aged 65 years or older. They also recommended a screening test for women under the age of 65 who are at risk for fractures. This test shows the strength of the bones so that preventative measures against fractures can be started if necessary.


Estrogen-containing oral contraceptives restore menstrual regularity42 and prevent the development of endometrial hyperplasia and endometrial cancer.43 A placebo-controlled randomized trial of estrogen-containing oral contraceptives involving women with dysfunctional uterine bleeding showed that 80% of the participants in the treatment group had improvement in their bleeding pattern compared with those in the placebo group, although this study was not focused on women over 40.42 Unfortunately, few studies have examined the use of oral contraceptives specifically for perimenopausal bleeding. One study including 132 perimenopausal women showed that estrogen-containing oral contraceptives reduced the risk of blood clots and heavy bleeding.44 Oral contraceptives were shown in a small randomized trial to reduce menstrual bleeding by 43%.45 Randomized trials involving women with heavy menstrual bleeding have shown the efficacy of an oral contraceptive containing estradiol valerate and dienogest, and of a combined contraceptive vaginal ring in treating this condition.46,47 However, this oral contraceptive is not currently available in Canada. Furthermore, observational studies have shown that oral contraceptives can reduce menstrual blood loss and increase hemoglobin concentrations, and their use is supported in clinical practice guidelines.48,49


The use of the levonorgestrel-releasing IUD has been proven effective in treating heavy menstrual bleeding, including when it is associated with adenomyosis and leiomyomas.18,48,50 The levonorgestrel-releasing IUD is licensed in countries including the US, the UK and Canada for the treatment of heavy menstrual bleeding. Its use leads to a 97% reduction in menstrual blood loss by 12 months and has high satisfaction rates.51 Although irregular bleeding can occur initially, amenorrhea rates of 20% to 80% have been reported at 12 months.49,52 A systematic review and meta-analysis found the levonorgestrel-releasing IUD to be as effective as endometrial ablation in reducing heavy menstrual bleeding.53 In addition, a clinical trial involving women with heavy menstrual bleeding showed that the levonorgestrel-releasing IUD was comparable to hysterectomy in improving hematologic parameters and quality of life.54


Although a 2011 meta-analysis showed a significant risk reduction of ovarian cancer among women with BRCA1 and BRCA2 mutations who had ever used oral contraceptives,67 the evidence regarding the effect of oral contraceptives on the risk of breast cancer among women with these mutations was inconsistent.67,68 It appears reasonable for women with BRCA1 or BRCA2 mutations and no personal history of breast cancer to use oral contraceptives; however, the risks and benefits should be weighed by the woman and her physician.69


  • Women ages 40 to 44 should have the choice to start annual breast cancer screening with mammograms (x-rays of the breast) if they wish to do so.

  • Women age 45 to 54 should get mammograms every year.

  • Women 55 and older should switch to mammograms every 2 years, or can continue yearly screening.

  • Screening should continue as long as a woman is in good health and is expected to live 10 more years or longer.

  • All women should be familiar with the known benefits, limitations, and potential harms linked to breast cancer screening.

Women should also know how their breasts normally look and feel and report any breast changes to a health care provider right away.


The American Cancer Society recommends that at the time of menopause, all women should be told about the risks and symptoms of endometrial cancer. Women should report any unexpected vaginal bleeding or spotting to their doctors.


  • Stay away from all forms of tobacco.

  • Get to and stay at a healthy weight.

  • Get moving with regular physical activity.

  • Eat healthy with plenty of fruits and vegetables.

  • It's best not to drink alcohol. If you do drink, have no more than 1 drink per day for women or 2 per day for men

  • Protect your skin.

  • Know yourself, your family history, and your risks.

  • Get regular check-ups and cancer screening tests.


By 50, generally, some collagen has left the lips. Fine lines and feathers may have begun. If you are or were a smoker, you might be seeing some of the effects on the lips. By age 70, you might describe your lips as mature wrinkled lips.


Bright, dark, or red lipstick is not the best lipstick for mature lips. There are exceptions to this generalization. Sometimes I see a fabulous older woman who pulls off a bright red lip or a bold color, and yes, it makes a statement.

Older adults are working longer. By 2018, 24 percent of men and about 16 percent of women ages 65 and older were in the labor force. These levels are projected to rise further by 2026, to 26 percent for men and 18 percent for women.3


The gender gap in life expectancy is narrowing. In 1990, a seven-year gap in life expectancy existed between men and women. By 2017, this gap had narrowed to five years (76.1 years versus 81.1 years).6


More older adults are divorced compared with previous generations. The share of divorced women ages 65 and older increased from 3 percent in 1980 to 14 percent in 2018, and for men from 4 percent to 11 percent during the same period.10


There are specific times when you should see your provider or receive specific health screenings. The US Preventive Services Task Force publishes a list of recommended screenings. Below are screening guidelines for women age 65 and older.
